Active Membership:

HOW TO APPLY

• Complete "Membership Application" form and return to SDDS OR apply online.
• Provide a wallet-sized photo for use in the Annual SDDS Membership Directory, either with your application or via email.
• Membership with SDDS is part of your tripartite membership (SDDS, CDA and ADA). Only one application is necessary.

MEMBERSHIP DUES

Membership dues are based upon a variety of factors including, but not limited to, date of graduation, ADA membership while in dental school and application date. Discounted dues are available for those enrolled in
a post-graduate program, full-time faculty, retired dentists and military/public health personel (specific qualifications apply).

Upon application, a dues statement will be created for your first year of membership. Prompt payment of your first year's dues will expedite your application process. All subsequent dues statements will be generated by CDA annually.

Dual Membership:

Dual Membership dues are $320 per year, and may be pro-rated to $160 after June 30. Discounted dues are available for those enrolled in a post-graduate program, full-time faculty, retired dentists and military/public health personel (specific qualifications apply).

Affiliate Membership:

Note: Dental students are eligible for Student Membership with SDDS while enrolled full-time in an accredited dental school. Students may convert their Student Membership into Active Membership and time during the year of their graduation. If Student Membership dues were paid for that year, they will be applied toward the dentist's dues for Active status.

DHP Membership:

DHP Membership dues are $95 per year.

Note: Dental Health Professionals (hygienists, assistants, office workers, lab technicians, county agency representatives, ect) are eligible for DHP membership if NOT affiliated with a company or individual who may otherwise be considered "vendors.".

Vendor Membership:

• Complete "Vendor Member Contract" and return to SDDS.

Vendor Membership dues are $3900 per year (Vendor A) or $2900 per year (Vendor B).
